If you are eligible for the service we aim to contact you within 5 working days of the team receiving your referral.
You will be contacted via telephone by a Cardiac Rehab Assistant who will explain the service offer, provide some initial advice and obtain your consent. This is your opportunity to ask any questions related to the service.
If you accept the service, we will book your initial appointment with a Specialist Clinician who will be able to tailor a Cardiac Rehabilitation programme that is personalised to meet your individual needs. This can be arranged via telephone or MS Teams video call. To prepare for this there is some information below...
To prepare for your upcoming clinician appointment please have ready:
- A list of your current medications you are taking.
- Any planned Cardiology appointments or investigations.
- A list of any questions or concerns that you have.
- Consider any lifestyle changes you wish to make. A guide can be found in your Cardiac Rehabilitation Booklet in the Risk Factor Management Chart.
- Any goals you may have, or activities you may want to return to.
- Complete any pre-assessment questionnaires sent to you prior to your appointment.
