What we do

The Heart Failure Team provides a specialist service to patients with a confirmed diagnosis of heart failure and signs and symptoms such as swollen ankles and feet and breathlessness.

The aim of the service is to improve quality of life by managing patients' healthcare needs in their own homes. Care is delivered either at home where indicated, remote monitoring or telephone consultations. We will help to manage symptoms, explain how to monitor the condition and ensure patients are treated with the best combination of evidenced based medication.

We work closely with those who are involved in the patients care including consultant cardiologists, GPs and other community based services, including the cardiac rehabilitation service, with the aim of providing  seamless care and support to patients, carers and their families.
