Personalisation
Cardiac rehab aims to make every patient journey personal, because we know that everyone’s recovery will be different. Every person is unique and so patients may have different goals and wishes to return to. For example, one patient may wish to return to competitive sport, whereas another may wish to return to gardening. These are equally important to that person and so it is important for us to tailor your recovery depending on what your needs are.
The NHS personalisation agenda supports us to do this through:
- Increasing the influence of the patient in decision making
- Giving the patient greater choice
- Giving the patient more information
We have tools in place to support your decision making about your care. All members of the team use a personalised approach to care through shared decision making. This means that the team may ask you “what do you think about this?” and “what would you like to do?”.
We work together with you to facilitate your care and enable you to reach your goals. By empowering you to understand your health condition, you are more likely to be motivated to self-manage your own care.
