People using exercise bikes and smilingOn completion of your cardiac rehabilitation, all the risk factors discussed at your initial consultation and during any reviews will be reassessed, ensuring that there is a ‘long term plan’ moving forward.  

Each patient will be encouraged and supported to develop self-management skills and understand when to escalate any symptoms or concerns that they may have to the relevant teams; for example, symptoms/medications to GP/111/999 and support with lifestyle risk factors such as exercise would be the Cardiac Rehab service.  

We will notify your GP of the discharge from the service to ensure that your blood pressure, heart rate, cholesterol levels, diet, blood sugar levels, alcohol consumption and any tobacco/drug use are reviewed and monitored at least once a year at the practice; although we do encourage patients to monitor their own observations if they are able to and ae able to provide support for this.  

The cardiac rehab team are also able to signpost you towards appropriate groups on discharge, such as:

  • Your local heart support group 
  • Community exercise and activity groups 
  • Community weight management services 
  • Smoking cessation services 
  • Psychological (mental health) support services

Please be assured, following your discharge from the Cardiac Rehab service, if you need any further advice, guidance and support relating to your heart, we welcome any queries for up to 6 months post discharge.  

If you require any ongoing support after discharge from the service, we are happy to accept either a self-referral back into service or a referral completed by a clinician whether this be at the GP surgery or a Cardiologist either via email at LHNT.CardiacRehab@nhs.net or via telephone on 01522 449900.
