We need to ask your permission to add your email address and NHS number into the system we use. The email will be sent from a website called Recap Health .
When you receive the email from Recap Health, you’ll see a link in it. You'll need to click on that link, then enter your name and choose a password. This is just a step so we know it's you who is viewing the information. After you've done that, you'll be able to see all the information we’ve picked out for you.
If you don't see the email it’s a good idea to check your email's spam or junk folder, just in case it's ended up there by mistake.
You can look at this information as many times as you need, and if you find you want more information later on, we can send that to you as well.
